- Professional or Employment Related Information such as current or past job history or performance evaluations.
- Non-Public Education Information (per the Family Educational Rights and Privacy Act (20 U.S.C. Section 1232g, 34 C.F.R. Part 99)) such as education records directly related to a student maintained by an educational institution or party acting on its behalf, such as grades, transcripts, class lists, student schedules, student identification codes, student financial information, or student disciplinary records.

- Information Received on your Behalf. You may authorize us to obtain information, on your behalf, from other third-party sources. For example, if you submit claims to the Centers for Medicare and Medicaid Service (“CMS”) you may decide to authorize us to obtain information directly from CMS. - Information Received as a Business Associate. Upon your request or with your consent, your health care provider (“Provider”) may share information about you, including your Personal Information, with Roudner PA. Roudner PA will only use such information as a “business associate” of a “covered entity” in accordance with any instructions or restrictions provided to Roudner PA by your Provider. - Use of Information Collected By Roudner PA. Roudner PA uses the Personal Information collected in an effort to improve your experience with the Service, to provide services to you and to communicate with you about information that you request.
